Services may be paused or suspended for non-payment .
Payment Terms
Private Pay services are billed biweekly. Payments may be made via ACH/bank transfer or other approved payment methods. Payment is due upon receipt of the invoice unless otherwise stated in writing.
Late Payments
Invoices not paid within 7 days of the due date may be subject to a $25 late fee. Continued non-payment may result in suspension or termination of services.
Authorization
By providing payment information, you authorize Elite Angels Healthcare to process payments for services rendered according to the agreed billing schedule.
No Refunds
Payments are non-refundable for services already provided.
Service Changes
We reserve the right to update service rates, billing practices, or these terms with reasonable notice.
Compliance
Services are provided in accordance with applicable state and federal regulations.
